HOW TO GET STARTED & OUR PROCESS

2. HAVE YOUR DOCTOR SEND US A REFERRAL. All skilled services such as physical therapy and nursing have to be done under the supervision of a physician that you are seeing so they have to fax us at 214-441-0291 the following items: This can be done by your PCP or any specialist seeing you.
Medical order for home health:
The order can read, “Home health to evaluate and treat.”Copy of visit note from an encounter that occurred within the past 90 days.
Your demographics which includes your current insurance information.
3. WE CHECK YOUR INSURANCE ELIGIBILITY TO ENSURE YOU HAVE HOME HEALTH BENEFITS AND THAT WE CAN ACCEPT YOUR INSURANCE FOR SERVICES. Every plan and policy is different. We have to look at your specific policy to ensure you have coverage for the service you’re needing. If for some reason we are unable to take your insurance, we will help find you another option so you can still get the services you need.
4. WE SCHEDULE AN EVALUATION WITH OUR NURSES AND THERAPISTS TO DETERMINE YOUR TREATMENT PLAN. A nurse and/or therapist will call you to schedule an evaluation to determine your treatment plan and will set a weekly/monthly schedule with you accordingly.
